Health inventory and immunization records
All newly admitted students residing in student housing must complete the health inventory
and immunization form before arriving on campus.

Students can obtain vaccinations and other tests at the Student Health and Counseling Center after they arrive. If an exemption is needed, please visit our forms page for exemption forms.
students can sign in using student id and password to upload all immunizations any other documents needed under the Immunization Upload feature. Students can update medical information and view immunization records and chart notes that SHCC currently has on file.
Access to your student's health information
When your student fillls out a Consent for Release of Protected Health Information
form, it allows our facility to share protected health information with parents. Without
your student's signed permission on this form, our center cannot provide you with any information
about your student’s health care or billing concerns as stated in our Notice of Privacy Practices.
Note: The release form is not a medical consent form.
